Event & Funding Requests
Event and Special Request Form
Student organizations planning to host an event on campus must submit their request via the Student Organization Event and Special Request Form. The Office of Student Engagement receives the completed form and uses the information submitted to request the event space, any IT Needs, and the facilities set-up. The Undergraduate Budget Committee request form for event funds is embedded in this form.
Transportation Request Form
Student organizations planning to host an event off campus must submit a transportation request form.
Facilities Request Form
Student organizations planning to host an event that needs a room set up, please fill out this form and attach it to the event and special request form.
Post Undergraduate Budget Committee Funding Form
Student organizations and individuals who are granted UBC funds must complete this form no later than five days after their event or travel is complete.
Update Student Organization Information
Officer Listing Form
Student organizations must submit an updated officers listing form at the beginning of the Fall semester and any time those holding their officer positions change.
Student Organization Advisor Agreement Form
Student organizations that change advisors must have their new advisor review and sign this agreement form.
Student Organization Social Media Information Form
Student organizations should enter their social media information. This will help the Office of Student Engagement promote your events.

Starting a New Student Organization
New Student Organization Application
Students starting a new student organization must complete the New Student Organization Application. The application includes the officers listing form, advisor agreement form, constitution, and interested students form. The completed application must be submitted to the Office of Student Engagement in the Carriage House or emailed to (StudentEngagement@chatham.edu).

Students who are interested in starting a new organization and would like assistance should complete this form.

Miscellaneous Information
Recognized Student Organizations are allowed to reserve and paint the Spirit Rock for promotion of their event, meeting, organization, etc. Use this form to reserve and request the supplies.
